As follows from the explanatory note to the draft budget law published in the State Duma database, funding for the federal project to combat oncological diseases in 2027 will be reduced to 3.873 billion rubles from 44.896 billion this year. In the 2025 budget, 148.46 billion rubles were allocated for it, so over two years federal budget spending on cancer fighting will decrease 38-fold.

The project funds the purchase of equipment for hospitals and dispensaries, medicines for cancer patients, and measures to develop early diagnosis. For 2028, the government is allocating 5.761 billion rubles, and for 2029 — 12.779 billion rubles; even at these levels, spending will remain below the current year’s level by 8 and 3.5 times, respectively.

Overall, the national project “Long and Active Life” will lose 20% of its funding next year: it will amount to 145.9 billion rubles versus 181.1 billion in the 2026 budget. The federal project “Development of Healthcare Infrastructure”, under which clinics and hospitals are built, repaired, and modernized, will receive 54.4 billion rubles instead of 77 billion.

Spending on the federal project “Sanitary Shield of the Country”, which is aimed at protecting the population from epidemics and infections, will decrease by 35% — from 14.1 billion to 9.2 billion rubles. Allocations to support healthcare in the occupied regions of Ukraine — the DPR, LPR, Kherson, and Zaporizhzhia regions — will be reduced from 8 billion to 522 million rubles.

State sanatoriums will receive 25.6 billion rubles instead of 37.4 billion, and under the item “medical and sanitary provision of certain categories of citizens” 90.5 billion is planned versus 108.7 billion this year. This category includes, among others, servicemen, disabled people, veterans of the Great Patriotic War, siege survivors, and those affected by radiation in Chernobyl.

Overall, federal budget spending on healthcare will decrease by 6% next year and amount to 1.818 trillion rubles. The Finance Ministry has cut medical spending for 2028 to 1.984 trillion rubles instead of 2.087 trillion in the current version of the law, while 17.1 trillion rubles, or 35% of all expenditures, is allocated for defense next year.
